NAME CHANGE REQUEST

inform you that the named JABO Ishimwe Kais, son/daughter of NISHIMWE Ben
Honore and UWIHIRWE Clemence Divine, domiciled at Ruganwa I village, Kinunga
cell, Gikondo sector, Kicukiro district, in the City of Kigali applied for the
authorization of the change of name from his current name JABO Ishimwe Kais to
be called JABO Ben Kais in the National Population Registry.
The
reason for the change of name is that It is a name given by my parents.
