NAME CHANGE REQUEST
We
inform you that the named IMENA ISHIMWE Koen, son/daughter of NISHIMWE Ben
Honore and UWIHIRWE Clemence Divine, domiciled at Ruganwa I village, Kinunga
cell, Gikondo sector, Kicukiro district, in the City of Kigali applied for the
authorization of the change of name from his current name IMENA ISHIMWE Koen to
be called IMENA Ben Koen in the National Population Registry. The reason for
the change of name is that It is a name given by my parents.
